End of life Care: The Latest Including Pain Management

- Read these instructions completely.
- View part 1 and part 2 of the Instructional Videos.
- Read the supplemental reading materials (15 minutes)
- Complete the Post-Test and the Evaluation (10 minutes).
- Submit credit card payment of $45 by completing the electronic payment form or pay by check by printing the form and submitting a check made payable to University Health Associates to the address indicated.
- You will receive a completion certificate through U.S. mail to the address you have indicated within two weeks of receipt of the evaluation and post-test (submitted automatically electronically) and the credit fee payment (submitted either electronically or via U.S. mail).
